Program Summary

The purpose of the Lifestyle Modification Program (LMP) is to prevent high risk people from developing type 2 diabetes.

Eligibility

Eligibility is defined as:

A person aged 40-49 years and Indigenous patients between 15-54 years with a high risk of developing Type 2 Diabetes as determined by the Australian Type 2 Diabetes Risk Assessment Tool

Patients must not have Diabetes or attended a previous LMP.

Under the national program, a GP can refer these patients to an LMP under Health Assessment item numbers:

Item 701

Brief – Attendance by a medical practitioner to undertake a short health assessment, lasting not more than 30 minutes

Item 703

Standard – Attendance by a medical practitioner to undertake a medium health assessment , lasting more than 30 minutes but less than 45 minutes

Item 705

Long – Attendance by a medical practitioner to undertake a long health assessment, lasting at least 45 minutes but less than 60 minutes

Item 707

Prolonged – Attendance by a medical practitioner to undertake an extended health assessment, lasting at least 60 minutes

Item 715

Aboriginal and Torres Strait Islander people’s health assessments for children, adults and older people

Practice Nurses and Registered Aboriginal Health Workers may assist GP’s in performing the Health Assessment.

The time that a Practice Nurse or Registered Aboriginal Health Worker spends in assisting with a Health Assessment may be added to the time taken by the GP to complete the assessment.

Program contents

 The content of the Lifestyle Modification Program must be delivered over 6-7 sessions and include:

  • The risks of diabetes and their relationship to lifestyle factors
  • The importance of regular diabetes screening
  • Nutrition advice and education
  • Physical activity advice
  • Behavioural strategies to support the adoption and maintenance of lifestyle change
  • Smoking cessation and alcohol reduction advice or referral if required
  • Information about community resources relevant to sustaining lifestyle change

Group size in any particular program will not exceed 15 clients

 

Cost

 The program is free for pensioners ands health care card holders. A total cost of $50 will apply to all other participants.

Program accreditation

Nationally consistent service delivery across all LMPs will be facilitated by an accreditation process based on agreed, evidence based LMP service standards.

GP feedback

 Facilitators must provide a report back to the participant's GP on program completion

As of 1 February 2010, all referrals to Diabetes Prevention programs (LIFE & LMP) should be sent directly to thge Program Facilitator of choice.
