Health assessment items

 

Program Summary

The health assessment item numbers involve an assessment of the patient's health and physical, psychological and social function and a determination on whether preventative health care and education should be considered. It includes:

  • Measurement of blood pressure, pulse rate and rhythm
  • Medication review
  • Continence assessment
  • Immunisation status
  • Physical function (assessment of independent daily living and falls risk)
  • Psychological function, including cognition & mood (measured by a recognised tool)
  • Social function and involvement of carers

There are a range of health assessment item numbers including for the Aged, (including residents of an aged care facility); for 45 - 49 year olds; for Aboriginal and Torres Strait Islanders; for Refugees and for the intellectually disabled. Medical literature support the following additional components: multi system review; fitness to drive; hearing; vision; oral health; diet and nutritional status; smoking & alcohol; foot care; sleep; home safety

The information collection may be undertaken by a nurse or assistant acting under GP supervision. The other components must include a personal attendance by a medical practitioner.

The assessment must include keeping a written record of the assessment, signed by the patient and provision of a written report to the patient.

The time period includes the time taken by the doctor and the practice nurse to undertake a health assessment.

Health Assessment for refugees & other humanitarian entrants by GP in consulting rooms conducted within 12 months of them arriving or receiving residency (whichever is the later).

 

One occasion only

Health Assessment for refugees & other humanitarian entrants by GP outside of consulting rooms, hospital or residential aged care facility, conducted wwithin 12 months of arriving or receiving residency (whichever is later).

 

One occasion only

Health Assessment for patients who are aged 45 to 49 years (inclusive) who are at risk of developing a chronic disease.

Once only for an eligible patient

A type 2 diabetes risk evaluation for people aged 40-49 years (inclusive) with a high risk of developing type 2 diabetes asdetermined by the Australian Type 2 Diabetes Risk Assessment Tool.

Once every three years to an eligible patient 

Health assessment of a patient with an intellectual disability at consulting rooms

1 per year

Health assessment of a patient with an intellectual disability at patients home

1 per year
