Program SummaryA case conference is a discussion where there are at least three members of a multi-disciplinary team communicating at the one time for the duration of the conference, either face to face, by telephone, video link or a combination thereof. The case conference involves the multi-disciplinary team undertaking the following activities :
A GP can organise or participate in a case conference in the community, aged care facility or for a private patient in hospital. A GP may only particpate in a case conference conducted on discharge from a public hospital. Patient consent is required whether the GP is organising or particpating. The GP is required to record day, times, names or participants, matters discussed and to provide a summary to the patient and team members. It is expected that a patient would not require more than 5 case conferences in a 12 month period. Allied Health Professionals who may participate (may also include one other medical practitioner):Aboriginal health worker; Audiologist; Community nursing; Daycare; Dental; Dietitian; Education providers; HACC - home help; Meals on wheels; Home maintenance; Occupational therapist; Optometrist; Orthoptist; Orthotist; Personal Care; Pharmacist; Physiotherapy; Podiatrist; Probation officer; Prosthetist; Psychologist; RDNS/ Registered nurse; Respite care; Social worker; Specialist Speech therapist; Transport.
Updates/Latest developmentsCurrently the case conference item numbers are under review with the intention of reducing their complexity. Findings of the review are scheduled for release in July 2009.
