Chronic Disease Management Network
The Chronic Disease Management Network (cdmNet) by Precedence Health Care is a web-based productivity tool for GPs, care providers and patients to assist them better manage chronic disease.
cdmNet automates the complex administrative and communication tasks associated with chronic disease management, freeing up GP time and improving practice nurse productivity.
Benefits of cdmNet
CdmNet offers a number of key benefits to GPs:
1. Supports high quality best-practice disease management
CdmNet support best-practice care for people with chronic illness (Wagner E, Austin B, Von Korff M. Organising care for patients with chronic illness. Millbank Q 1996: 511-544)
- Facilitates individual patient care planning
- Shares information with patients and providers to coordinate care
- Tracks the activities of the care team and patient
- Provides timely reminders for providers and patients
- Ensures regular follow-up by the multidisciplinary care team
2. Improves productivity and reduces administrative burden
CdmNet simplifies care planning and review and automatically manages Medicare Chronic Disease Management (CDM) requirements:
- Creation of GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
- Follow-up and Review of GPMPs and TCAs
- Distribution and coordination of TCAs and TCA Reviews
- Completion of Annual Cycles of Care and Home Medication Reviews
- Generation and distribution of forms and documents, such as Allied Health referrals
3. Removes barriers to increased revenue using eisting resources
CdmNet enables a GP to generate up to $75,000 per year from Medicare CDM Items:
- More patients on care plans (GPMPs and TCAs)
- More CDM items per patient (GPMP/TCA Reviews and Annual Cycles of Care)
- With no increase in time burden on the GP or practice nurse
CdmNet is easy and fast to implement with no new application to install on the health care providers desktop computer. It requires minimal training and is simple to use.
CdmNet manages diabetes, osteoarthritis, coronary heart disease, chronic heart failure, stroke, chronic kidney diesease, asthma and chronic lung disease. It also manages depression as a co-morbidity with orther chronic diseases.
CdmNet Trial
CdmNet is being trialled in an $8.7 million project in the Barwon South Western Region under the title CDM-Net (Chronic Disease Management Network). Up to 3000 patients with Type 2 Diabetes (together with their GPs, care team and the geelong Hospital) are expected to participate in the trial. CDM-Net is supported by funding from the Australian Government under the Clever Networks program, the Victorian Government, the Barwon Medicare Local, Barwon Health, Deakin University and other world leading industry and research partners.
The trial is due for completion in October 2009.
