STRATEGIC THEME ONE:             Strengthening General Practice for a healthier Community

Our Goals

Our Objectives

Support for GPs personal and continuing professional development

To provide GPs access to peer support programs.

 

To provide continuing professional development activities in response to local GP needs.

To provide opportunities for networking with colleagues and other health care professionals

Through leadership, enhance GPs working relationships with key stakeholder groups.

To establish GP leadership networks (GPLN) that offer opportunities for a shared understanding on key areas for improvement

 

 

To actively contribute to the Barwon Health GP Liaison program

Identify and support workforce, e-health and infrastructure capacity for current and emerging general practice models

To provide General Practice with support and resources to assist with General Practice recruitment

 


 

To support practice nurses, practice managers and allied health professionals with systems and education that can best support the role of the GP

To be a national leader in the delivery of electronic communication that promotes efficient, quality and safe clinical practices

To assist General Practice in responding to opportunities for practice extensions that support teaching and multidisciplinary team care

Enhance the management and quality of General Practice

To provide General Practice with resources to undertake a review of their models.

 

 

To explore opportunities to contract allied health services into General Practice

To deliver benchmarking and quality improvement initiatives to General Practice.

Deliver allied health services that fill service gaps in the community.

To provide either directly or in partnership, quality allied health services to young people, culturally and linguistically diverse, low socioeconomic, aged and indigenous groups.

 

 

STRATEGIC THEME TWO:            Strengthening Communities in partnership with General Practice

Our Objectives

Inform local population health planning and priorities for the region

To provide General Practice with local data and broader community plans to identify and respond to service gaps and risk factors.

 

 

To support General Practice in providing advocacy for their communities via G21 regional planning

To participate and influence health and wellbeing priorities for the region through the provision of General Practice data

Increase the community’s awareness and access to primary care services

To promote awareness of the value of immunisation in the community

 

 

To increase access to primary care services for the indigenous community

To increase access to primary care services for adolescents in the community

To enhance systems for GPs to support their Aged Care residents and After Hours care for their patients

Promote services that enhance health literacy in the community

To work collaboratively with local schools in promoting a stronger link between education and primary health care services

 


 

To support General Practice in Advanced Care Planning and palliative care

To work collaboratively with the Early Years sector and General Practice to enhance family health outcomes

To promote positive health messages and the role of General Practice to the community

To promote the uptake of personalised electronic health records in the community

Enhance governance practices and framework

To have sound stewardship of GPAG financial resources and corporate governance

 

To regularly review board governance and performance.

To actively source alternative funding streams which support our Strategic Plan

To ensure there is open transparency between GPAG and the members of the company

Inform and lead local health system reform through partnerships

To position GPAG and General Practice as a future leader in primary health care reform

 

 

To engage with General Practice and stakeholder groups to collaborate on future system changes

Monitor, evaluate and improve our performance

To use quality, evidence based data to support the services provided through GPAG

 

To review and respond to the emerging needs of GPAG communities of interest

To ensure GPAG engages in continuing quality improvement activities.

To ensure staff can openly give feedback to management on opportunities for improving the work place

 

To develop recruitment and orientation processes which support GPAG staff

To provide a Performance Development Program which supports staff identifying and accessing professional development to achieve their professional goals
